Meet The Physicians

Peter D. Cladis, M.D.

Dr. Peter Cladis joined our practice in July of 1983. He graduated Summa Cum Laude from Denison University in Granville, Ohio, and received his M.D. degree from Northwestern University Medical School in 1980. His residency was served at the Cedar Rapids Family Practice Residency Program where he was Chief Resident. Dr. Cladis is Board Certified in Family Practice and has successfully completed continuing education to qualify for recertification on three occasions.

A Chicago native and avid White Sox fan, Dr. Cladis and his wife, Jane, are residents of Geneva and raised their three children, Dennis, Andrea and Stacey here. They have been active community leaders in school, park district programs and administration, sports, music and are enthusiastic supporters of the arts.

Dr. Cladis has been involved in hospital governance, having served as medical staff president, and currently serves as chairman of the Department of Family Practice at Delnor Hospital. He regularly volunteers his time serving patients at Tri-City Health Partnership, a free clinic for Fox Valley residents who are unable to pay for their medical services.

Paramount to his style of medical practice is a special focus on patient education and the patient’s complete understanding of their particular medical issue. Concern for wellness and preventative medicine emphasizing nutrition, exercise, and emotional health is a strong practice interest. Dr. Cladis enthusiastically embraces the entire family practice patient spectrum from infants to the elderly, and has extensive experience in the areas of women’s health, pediatrics, and minor surgical procedures. He is dedicated to the philosophy that his medical practice provides compassionate, consistent and comprehensive care.

Michael J. Fremgen, M.D.

Dr. Michael Fremgen graduated Cum Laude from Marquette University in Milwaukee and received his M.D. degree from the Chicago Medical School in 1982. He completed his residency program at the University of Illinois in Rockford. Dr. Fremgen joined our practice in the summer of 1985.

Joseph M. Neubauer, M.D.

Dr. Joseph M. Neubauer is a Board Certified Family Physician who has been in practice for 25 years. Raised in Schaumberg he attended the University of Illinois as an undergraduate. After undergraduate school he attended Rush University Medical School, where he received his medical degree. Dr. Neubauer served his residency at the Lutheran General Hospital Family Practice Residency Program. He joined Fox Valley Family Physicians in July 1987. Dr. Neubauer and his wife Dawn currently live in Geneva.

Thomas R. Hazel, M.D.

Dr. Thomas Hazel is a Board Certified Family Practice Physician, and has been in practice for 22 years.  Dr. Hazel joined Fox Valley Family Physicians in 1990 when he and his wife, Anita, move to Geneva from Oak Pak, Illinois.  A graduate of Michigan State University’s College of Human Medicine, Dr. Hazel completed his internship and residency in Family Medicine at West Suburban hospital in Oak Park. He served as Chief Resident in his senior year of residency.

As a native of the Midwest, Dr. Hazel enjoys the strong sense of family, church and community he has experienced in the Tri-cities area.  He and his wife are raising two daughters.  His youngest daughter attends Geneva High School and the oldest is a freshman and Wheaton College.  In his free time Dr. Hazel enjoys spending time with his family, playing tennis, going for walks with his wife and their two dogs, as well as reading mystery novels and comic books.

In 2003 Dr. Thomas Hazel wrote, illustrated and published a children’s bedtime picture book, “Eastwood Stop Barking! There’s Nothing Out There!”.  The Hazel family attends church regularly where Dr. Hazel has been active with Sunday school and the High School Youth groups.

Dr. Hazel enjoys providing comprehensive primary care for all age groups.  He approaches both well and sick visits with equal enthusiasm, striving to keep patients healthy or to quickly return them to health.  In all interactions he believes communication between doctor and patient is of the utmost importance.  He believes patient care is enhanced by a more personal doctor-patient relationship.  He sees his practice as a ‘medical home’ where patients can seek and receive excellent and compassionate medical care, get answers when they don’t know which way to turn, or be referred to the proper specialist.  He believes in overseeing all aspects of a patient care.

Katherine G. Fackler-Chapman, M.D.

Dr. Katherine Fackler-Chapman is a Board Certified Family Physician who has been in practice since 1989.  Dr. Fackler joined Fox Valley Family Physicians in 1996, after practicing in the US Air Force for 7 years, including teaching at  the Family Medicine Residency at Eglin AFB from 1992-1996.  A graduate of Ohio State University College of Medicine, she completed her internship and residency at RL Thompson Strategic Hospital at Carswell AFB, Texas.  Dr. KFC’s undergraduate degree is from Valparaiso University in Valparaiso, Indiana.

An Ohio native, Dr. KFC moved to Geneva to return to the Midwest.  She and her husband wanted to raise their 2 children in a town with a strong sense of community.  Dr. KFC feels that sense of community helps support patients and family.  In her free time Dr. KFC enjoys bicycling with her husband, playing hand bells and singing in her church choir, reading, and working at the Tri-city Health Partnership.

Dr. KFC believes in partnering with her patients to maintain their health, encouraging them to take an active role in their health maintenance and supporting them in a healthy lifestyle.  She enjoys seeing patients from infants to elders, and feels the family medicine approach helps support the health of individuals and the family itself.

Tony Leazzo, D.O.

Dr. Tony Leazzo joined our practice in August of 2017, coming to us with over 12 years of medical experience. He graduated from Gannon University in Erie, Pennsylvania, and received his D.O. degree from Philadelphia College of Osteopathic Medicine School in 1998. His residency was served at the Genesys Regional Medical Center Family Medicine Program. His residency was interrupted by military service in the United States Navy where Dr. Leazzo served as a General Medical Officer for 3rd Battalion 5th Marines and 1st Fleet Service and Support Group at Camp Pendleton in California.

After completing his Family Medicine Residency, Dr. Leazzo went on to complete a one year fellowship in Sports Medicine at Michigan State University. Dr. Leazzo is Board Certified in Family Medicine, Sports Medicine and Occupational Medicine.

Dr. Leazzo has been involved in hospital governance, having served in many roles on the medical staff and as vice-president of the medical staff for two years. Additionally Dr. Leazzo has co-authored a chapter on shoulder disorders in the Osteopathic Clinical Joint Exam textbook and recently published an article on Obstructive Sleep Apnea. Besides writing, Dr. Leazzo is one of the team physicians for U.S.A. Soccer, having traveled to France, Mexico and Portugal.

A Cicero native and avid Cubs fan, Dr. Leazzo and his wife, Jennifer, are residents of Aurora where they are raising their two children, Tony and Michael. They are active in their church community and are frequently seen running everywhere with their children’s sports and school activities. In his “spare” time, Dr. Leazzo enjoys Scuba diving and is learning to play the guitar.

Dr. Leazzo enjoys caring for the entire family practice patient spectrum from infants to the elderly, and places a high value on being at the patient’s hospital beside when the patient needs him the most. Dr. Leazzo strives to care not only for the individual patient, but the patient’s whole family as well in order to provide true Family medical care.

Stacey Brown, PA-C

Stacey Brown joined FVFP in 1999. A Summa Cum Laude graduate of Carthage College, she graduated the Valedictorian of her class at the Finch University of Health Sciences at the Chicago Medical School Physician Assistant master’s program.

Mary Beth Kunzer, FNP

Mary Beth Kunzer joined FVFP in September 2001. A 1996 BSN graduate from Millikin University, she graduated with distinction from DePaul University in 2001 where she earned her Master of Science in Nursing, Advanced Nurse Practitioner Degree.

Lynn Templeton, FNP

Lynn Templeton joined FVFP in July 2008. She received her RN degree from Harper Hospital School of Nursing in Detroit, Michigan, and her bachelor’s degree in Nursing from Oakland University. After completing her Master’s Degree in Nursing at Wayne State University, she completed her post-masters as a Family Nurse Practitioner at Arizona State University in 1994.

Cristy Steinmetz, FNP-BC

Cristy is our most recent addition to the practice and joined FVFP in June 2014. Following completion of her Bachelors of Science in Nursing degree at the University of Illinois in 2008 (Go Illini!), Cristy worked as a RN in the Intensive Care Unit at Delnor. She earned her Master’s of Science, Nursing degree, specializing as a Family Nurse Practitioner, from the University of Illinois at Chicago in 2014.

Lauren Miser, PA-C

Lauren Miser joined FVFP in December 2017. Though originally from New Jersey, she earned her bachelor’s degree from Wheaton College in Illinois. She then attended Midwestern University in Downers Grove for her Masters of Science in Physician Assistant Studies. Prior to PA school, she gained healthcare experience as a medical assistant in a cardiology practice and as a volunteer EMT. In her spare time, she loves to sing at her church, cook and bake, and play board games with her husband.
