FAQ’s

The following are responses to frequent questions from our patients. The information may be helpful to you.

New State Requirements for Children – There are two new state requirements in Illinois regarding your child’s health care and entry into school that became effective on January 1, 2008.

  • The first new law states that a physical examination is required for entry into sixth grade rather than fifth grade.
  • There is also a new state law requiring an eye examination for all children entering kindergarten, or any child new to the district in 2008. This eye exam must be done by an optometrist or an ophthalmologist and will therefore not be included as part of their exam here.

You may contact your school nurse for more information on these requirements and/or to obtain forms.
